What to submit

• Non-refundable processing fee of $75 and three (3) copies of all application materials:

• Completed questionnaire (typed)

• Supporting documentation (forms, handouts)

• Optional video (Certified only)

Where to submit.  Send application materials to the Administrator.  Do not use registered or certified mail or any delivery service requiring a signature.
